PA State Mandates

6th Grade students must have a physical and scoliosis exam during the 6th grade year. Please get an updated copy of your child's vaccine record from your child's primary care provider. A booster dose of tetanus, diphtheria, and pertussis (Tdap) and one dose of the meningococcal vaccine are required for 7th grade. Students who have not shown proof of Tdap and the meningococcal vaccine risk exclusion from school per the Pennsylvania Department of Health Guidelines. Please send your child's completed paperwork to Mrs. Sheerer by June 1.  Physical forms can be found under the home tab on the main page of the health services site.
