PA State Mandates

6th Grade students must have a physical and scoliosis exam during the 6th grade year. Please get an updated copy of your child's vaccine record from your child's primary care provider. A booster dose of tetanus, diphtheria, and pertussis (Tdap) and one dose of the meningococcal vaccine are required for 7th grade. Please send your child's completed paperwork to Mrs. Sheerer by June 1, 2021. Physical forms can be found under the home tab on the main page of the health services site.

7th Grade students must have a dental and a scoliosis exam during the 7th grade year. Please send your child's completed paperwork to Mrs. Sheerer by June 1, 2021. Dental forms can be found under the home tab on the main page of the health services site.

Any current 7th grade student that did not send in their 6th grade physical, scoliosis, or updated vaccine record must sent their completed paperwork to Mrs. Sheerer by March 29, 2021. Students who have not shown proof of Tdap and the meningococcal vaccine risk exclusion from school per the Pennsylvania Department of Health Guidelines. If your student is in the virtual option, the updated vaccines must be provided to the nurse. This rule applies to ALL North Pocono School District students!
